The Edinburgh Self Management Practitioner Toolkit has been developed by cross-sector practitioners who work alongside people living with long term conditions – it’s now ready for you to use and test in practice!

What is it?
An information resource bringing together key concepts and practice tools associated with Self-Management Approaches. Self-Management Approaches refer to ways of working which empower and enable people to take the ‘driver’s seat’ in managing their own health and wellbeing.

Who is it for?
Edinburgh practitioners using or looking to use Self-Management Approaches in their work with people living with long term conditions.

Practitioners from all sectors have expressed a need and a desire for a local resource which brings relevant information together in one place to support new starts and experienced practitioners looking to develop their Self-Management ways of working.

What does it look like?
A series of ‘bitesize’ Topic Guides around key concepts and aspects of Self-Management practice.Each includes an introduction to topics and how they fit with Self-Management Approaches, links to further information and peer support to develop understanding and connections, and practice examples sharing learning and experiences with the aim of providing peer to peer learning. To date 7 topic guides have been developed:

  • Getting into a Self-Management Mindset
  • Self-Management and asking What Matters to You?
  • Signposting to supporting Self-Management
  • Lifestyle Management supporting Self-Management
  • Chronic Pain and supporting Self-Management
  • Digital Technology supporting Self-Management
  • Self-Management Values and Principles
